Parent/Guardian Information:
     - name(s)

     - phone number(s)

2. Complete an essay
Essay Topic:
"If chosen as a Youth Tour delegate, you will be traveling to Washington, D.C. to experience and learn about America’s rich history. What moment in American history do you wish you had been a part of and what would you have contributed?"

Essay Format:
     - Not to exceed two standard 8.5x11-inch typewritten
     - Double-spaced
     - 12pt Times New Roman Font

3. Upload student photo (ex. school picture) for use in connection with the Youth Tour

4. Upload completed Consent and Release Form (parent signature required if under 18 years of age)
